Please use the forms linked below to notify WTA of any of the following: testing positive for Covid-19, symptoms, travel (domestic or international) or other possible exposure to COVID-19.

Case (You have tested positive for Covid-19)

Possible Case (You have symptoms but have not tested positive for Covid-19)

Household Member (A household member has symptoms or has tested positive for Covid-19)

External 1st-Level-Contact (You were a close contact with a confirmed case, outside of our premises)

Travel (International or Domestic)

Test Results (notify WTA of Covid-19 Test Results)
